A sliding-fee discount can lower what you pay for care here when your household income is limited. Many people who qualify never ask. Please ask. You will not be turned away for asking.

If your income is limited, you may pay a small set fee. Ask. It is policy, not a favor.

  1. 1 of 4 · Who can get a sliding-fee discount?

    Households at or below 200% of the federal poverty guidelines — and you should still ask if you are close.

    If your income is at or below the federal poverty level, you pay only a small set fee. If you cannot pay it, you will still be seen. We use the federal poverty guidelines, updated each year. You will be asked to share financial information about once a year so the discount stays current.

    The discount applies to Crystal River Health Center charges. It does not automatically change a specialist or hospital bill. Ask us which bills it covers.

  2. 2 of 4 · Does the discount apply if I have insurance?

    Yes. Insured patients can get the discount too.

    The discount applies to insured patients too. No one is denied care because they cannot pay. If nobody mentioned the discount, bring it up at check-in or billing.

    Ask even if you have Medicare or other coverage. Coverage and sliding fee can both be part of the picture.

  3. 3 of 4 · What do I need to bring?

    Proof of income — and questions.

    The desk will tell you what counts (pay stubs, a letter, or other proof). If you do not have paperwork with you, come anyway. We can start the visit and finish the discount when you return with it.

    A one-page sliding-fee sheet is available. Ask for it in English or Spanish.

  4. 4 of 4 · Is this the same as help with food or rides?

    Related, but not the same. Ask for both.

    Sliding fee is about what you pay us. Food, housing, and transportation are community supports we can help you find. You can ask for both in the same visit.
