1 of 4 · Who can get a sliding-fee discount?
Households under 200% of the federal poverty guidelines — and you should still ask if you are close.
We use the federal poverty guidelines, updated each year. You will be asked to share financial information about once a year so the discount stays current.
The discount applies to Crystal River Health Center charges. It does not automatically change a specialist or hospital bill. Ask us which bills it covers.
2 of 4 · Can my copay be zero?
Sometimes yes — when the policy says so.
When you meet the policy, there may be no copay. Staff are supposed to say that out loud. If nobody mentioned it, bring it up at check-in or billing.
Ask even if you have Medicare or Medicaid. Coverage and sliding fee can both be part of the picture.
3 of 4 · What do I need to bring?
Proof of income — and questions.
The desk will tell you what counts (pay stubs, a letter, or other proof). If you do not have paperwork with you, come anyway. We can start the visit and finish the discount when you return with it.
A one-page sliding-fee sheet is available. Ask for it in English or Spanish.
4 of 4 · Is this the same as help with food or rides?
Related, but not the same. Ask for both.
Sliding fee is about what you pay us. Food, housing, and transportation are community supports we can help you find. You can ask for both in the same visit.
