Continuum of Care

Therapy is the work of getting better: talking with a counselor, a medicine, equipment, or rehab or a procedure we connect you with. The test is not the order on paper. The test is whether it reached your house.

Counseling, medicines, and equipment, plus referrals for rehab and procedures — with help getting the bottle or walker home.

  1. 1 of 4 · What kinds of treatment do you offer?

    Counseling, medicines, and equipment — and referrals for rehab and procedures.

    Counseling and behavioral health sit beside your medical visit. When physical therapy or a procedure would help, we can connect you with it through a referral, often to a partner site. You can fill your prescriptions at any pharmacy you choose. Ask us if you need help paying for your medicines. Equipment might be a walker, a home scale, or a breathing machine.

    If we refer you out, we still want to know whether the first appointment happened.

  2. 2 of 4 · How do I get my medicines?

    Ask about medicine costs — and bring the bottles.

    You can fill your prescriptions at any pharmacy you choose. Ask us if you need help paying for your medicines. Bring every bottle, including over-the-counter pills and inhalers.

    We will not print milligrams on a flyer. Your pharmacist reads the label with you. If a pain-medicine plan is part of your care, naloxone (the overdose-reversal medicine) may be part of the conversation too.

  3. 3 of 4 · What if I cannot start therapy?

    Money and rides are treatment issues. Say them out loud.

    If a copay or a missing ride stopped the first physical-therapy visit, that is not “noncompliance.” That is a problem we can work on — sliding fee or a different time of day. Tell us if getting here is hard. We can help you set up a ride. Most rides must be booked 2–3 business days ahead.

    A one-page sheet can sit on the fridge while we sort the logistics. Ask us to send one.

  4. 4 of 4 · Who do I call if a treatment feels wrong?

    Us — the same week. Do not wait for the next annual visit.

    Call 352-565-7342. If you cannot breathe or have chest pain, call 911. Call 911 if you or someone else is in danger now. 988 if someone needs a person now.

    Bring the bottle or the equipment to the visit. It is easier to fix what we can see.
