A stack of papers rarely changes a week. We use short one-page sheets — 119 of them, in English and Spanish — and a person on your team walks through why it matters, what to do this week, and when to call.

  1. 1 of 4 · What will I actually receive?

    One page. Plain words. Then a conversation.

    Sheets cover things people in Citrus live with: blood pressure, diabetes, breathing, medicines, mood, pregnancy, falls, getting care. Each page says why it matters, what to try this week, a teach-back question, and when to call.

    You should be able to read it at the kitchen table. If you cannot, tell us. We will say it out loud.

  2. 2 of 4 · How does a sheet get to me?

    Text, email, print, or mail — from your team.

    Your CareCoach or clinician can send a sheet by text, email, print, or U.S. mail. You see the sheets that were given to you — not a whole library to browse alone.

    If you did not get a copy, ask. Bring it to the next visit and we will mark what you already tried.

  3. 3 of 4 · Will it tell me how many milligrams to take?

    No. That belongs on the bottle and with your clinician.

    Education sheets do not print dosing tables. Your pharmacist and your clinician read the label with you. If a medicine does not make sense, bring the bottle.

    Mood screens on these pages stay short. Counseling details stay in your medical chart.

  4. 4 of 4 · What if I still do not understand?

    Say so. That is the point of teach-back.

    Teach-back means we ask you to say the plan in your own words. If you cannot, we failed — not you. We try again in simpler words or another language.
