Your week, not only a spreadsheet.
Better can mean an A1C that is safer, blood pressure under a goal you and your clinician set, fewer panic nights, or walking farther without stopping.
We also watch whether you needed the emergency room or a hospital stay. Good care should make those less common — not more paperwork.
2 of 4 · What conditions are common in Citrus?
Weight, blood pressure, diabetes, and pain — often together.
Many neighbors live with high blood pressure, diabetes, extra weight, and chronic pain — especially as we get older. You are not the only one in the waiting room with more than one of those.
If you have several long-term conditions, you may see us more often. That is on purpose. Weekly check-ins are for people who need them, not a punishment.
3 of 4 · How will I know if I am on track?
Your year-long plan and your yearly visit.
If you have a chronic condition, your team keeps an annual plan: what to watch, when to come back, which sheet to keep on the fridge. A yearly wellness visit is a good time to reset that plan.
Ask us to show you your last A1C, blood pressure, or weight in words you understand. You should leave knowing one thing to do this week.
4 of 4 · What if I was just in the hospital?
Call us. We want to see you the same week.
After a hospital or ER visit, someone from our team should reach you quickly — papers on the table, old and new bottles side by side, a next appointment.
If nobody called, call (352) 565-7342 and say you were discharged. Bring the papers they sent you home with.
