1 of 4 · What can I get here?
Four pillars, one team.
Primary care — a doctor or nurse practitioner who knows you — is the center. Around that sit our in-house pharmacy (opened November 2025) so many medicines cost less, counseling and mental-health support for children and adults, and dental care with a special focus on kids.
If a piece is not in this building yet, we arrange it. You should leave with a next step, not a shrug.
2 of 4 · What happens after the visit?
A plan you can actually follow at home.
We look at what is going on (tests and check-ins), treat it (therapy, medicine, equipment), then keep you well over the year. If you have a long-term condition such as diabetes or high blood pressure, your team keeps a year-long plan so everyone is working from the same list.
A CareCoach is a person on your team who helps between visits — the sheet, the ride, the refill reminder — not a second doctor.
3 of 4 · Does this change as I get older?
Kids, adults, and older adults have different doors.
Children: we work to keep medicines affordable and dental care in reach. Adults: ask about sliding fee if money is tight. Older adults: a yearly wellness visit is the foundation, plus a plan if you live with more than one chronic condition.
Bring your medicine bottles. We will line them up with the list. We will not print dosing tables on a flyer — your pharmacist and clinician read the label with you.
4 of 4 · What about mood and crisis?
We will ask. We will not leave you alone with it.
A short mood screen (two questions) may be part of a visit. If it is positive, we connect you with a clinician. Counseling notes stay in your medical chart, not on a public page.
